Consent to Tele-Health
This Telehealth Informed Consent describes the nature of telehealth services made available through PeptidesQ, Inc. ("PeptidesQ") and the licensed clinicians who use the PeptidesQ platform. Please read it carefully and confirm your agreement before beginning a visit.
What telehealth is
Telehealth involves the use of secure electronic communications — including asynchronous intake forms, messaging, photo and document uploads, and, where clinically indicated, live audio or video visits — to evaluate, diagnose, treat, and follow up with patients remotely.
Benefits
Benefits may include faster access to a licensed clinician, more convenient follow-up, home delivery of medications when appropriate, and continuous documentation of your care.
Risks and limitations
Telehealth has limits. Your clinician cannot perform a hands-on physical exam and must rely on the information you provide. Technology can fail, information can be delayed, and, in rare cases, electronic systems can be breached despite reasonable security measures. If your symptoms are severe, worsening, or emergent, call 911 or go to the nearest emergency department.
Your responsibilities
You agree to provide complete and accurate personal, medical, and medication information, to update your clinician when your condition or medications change, and to follow instructions carefully. Do not share or resell prescribed products.
Privacy and records
Visits, messages, and documents are stored in a secure electronic health record. Access is limited to authorized clinicians and staff involved in your care. Handling of your information is described in our Privacy Policy and complies with HIPAA and applicable state law.
No guaranteed outcome
Your clinician will use professional judgment to determine whether treatment is appropriate. There is no guarantee that any specific diagnosis, prescription, or clinical outcome will result from a telehealth visit.
Right to withdraw
You may withdraw your consent to telehealth at any time by notifying your clinician or PeptidesQ in writing, without affecting your right to future in-person care from another provider.
Governing law
This consent is governed by the laws of the State of Florida. PeptidesQ, Inc. is located at 20900 NE 30th Ave, Ste 200, Aventura, FL 33180.
